This is a part-time 20-hour permanent post.

Grade 6+ 2 Increments - Pro Rata Salary £17,040 - £18,506.

We have an exciting opportunity to join our team of Care Leaders at 220 Preston Road.

This is a service that provides support to adults with a learning disability, complex needs, and challenging behaviors. If you have substantial experience of working in a care or support setting, a natural leader, and someone who is passionate about looking after others, then this role is for you.

We are looking for those with a professional, energetic approach and most importantly displaying a caring, passionate, and empathic approach. Someone who can manage a staff team on a shift and support the deputy manager and manager. You would be expected to work independently and use your own initiative.

As a frontline hands-on care leader, you will need to be able to lead a shift, oversee the day-to-day running of a responsive and innovative service, and you will be expected to:

  • Rota manage a team effectively
  • Promote and safeguard the welfare of adults
  • Supervise/Mentor staff
  • Conduct supervisions, performance, and probation reviews
  • Write and develop risk assessments
  • Facilitate best interest reviews
  • Oversee and monitor care and support plans
  • Ensure the homes policies and procedures meet the required CQC key lines of enquiries.
  • Complete records in relation to accident/incidents and complaints.
  • Ensure medication is administered as per policies and procedures and oversee stock control.
  • Lead by example

You will need to hold a Level 3 NVQ in health and social care or equivalent.

To lead, coordinate, and supervise the team in ensuring that what is important to the people is achieved through person-centered practice.

To support, assist, and deputise for the Deputy Care Manager in maintaining efficient and effective day-to-day operation of the home.

In the absence of the Deputy Manager and Registered Manager, lead the running of the Residential Unit.

The postholder will be expected to contribute to the continuous improvement, co-production, and development of the service to ensure it develops in-line with the needs and aspirations of the customers, the people it supports, their families, and Hull City Council, the strategic vision, and goals of Adult Social Care.

PRINCIPAL ACCOUNTABILITIES:

  1. To promote and safeguard the welfare of children, young people, and/or vulnerable adults.

  2. Statutory Obligations - Undertake duties and responsibilities in accordance with the national minimum standards of care issued by relevant bodies e.g. Care Quality Commission (CQC). To provide direct support, guidance, supervision, and care which meets the persons' assessed needs in accordance with their agreed support plan.

  3. Develop and implement risk assessments relating to individuals' care and support, taking into account the physical and emotional needs of the people we support. Prepare moving and handling risk assessments for individual people and direct staff on safe methods of transfer that comply with current legislation and preserves the dignity of people who use the service. Setting up a provision and completing initial paperwork and consent to care tasks. Liaising with the person, their families, and carers, and other professionals. Taking the lead on standard transitions into the service, referrals for Transport, and ensuring the correct person-centered information is included on the care and support plans.

Oversee and monitor the completion of care and support plans, including the completion of monthly summaries, and consulting with others to make the necessary changes to individual care and support plans.

  1. To direct, support, and lead Care Workers in the provision of residential services for people with a range of disabling conditions, ensuring that the physical and emotional needs of the people the service supports are met, whilst always promoting person-centered approaches, dignity, privacy, and personal choice. Tasks include leading, directing, supporting with, and undertaking personal care including continence care, washing, dressing, oral hygiene, assisting with eating and drinking, PEG feeding, postural management, catheter and stoma care, and support and direction with following prescribed diets to ensure safe and adequate intake of nutrition and hydration.

Ensuring safe administration of medication through providing supervision and direction to staff undertaking medication duties through observations, spot checks, MRC audits, and training.

To ensure that medication is administered as prescribed, by the medical practitioner, and as per risk assessment/health action plan and universal medication policy, through various routes i.e. oral, topical, and peg, and appropriate records are accurately maintained by Residential Care Workers.

Takes the lead in ordering, maintenance, and safe disposal of medication stock (including creams and medical appliances).

To lead in the support of people with behaviors of concern in line with the persons' care and support plans, following all relevant legislation, physical intervention training, good practice guidance, and least restrictive principles.

  1. Take the lead in and hold staff briefings, ensuring the fair delegation of tasks and responsibilities, and the handover of service-critical and relevant health and safety information is completed daily.

To be responsible for allocating, organizing, and prioritizing work and supporting Activity Organisers to develop programmes of activity, including identifying and sourcing internal/external activities which meet the needs of the people who attend and the services.

Making decisions and problem-solving day-to-day issues to ensure smooth running of the service, such as changing activity rotas, staff rotas, and staff deployments due to sickness or other absence. Making amendments to individual services as necessary and deciding whether escalation is required.

  1. To set up and maintain effective communication between a range of health and social care colleagues, carers, and families, making referrals when appropriate to other agencies or council departments and maintaining effective communication with the people we support, their carers, and their families.

Attend multidisciplinary meetings with stakeholders for the purpose of best interest outcomes for the people we provide care and support to. Write and coordinate reports/information for individual reviews including liaison with other professionals/family, friends, and carers. To follow up and track all actions are taken and take further action if necessary.

As a Keyworker, facilitate person-centered reviews to promote the individuals' agreed outcomes. Work closely with the people we support, their families, and other professionals to review plans on a regular basis to ensure consistency and to maximize the effectiveness of the provision.

To contribute to and the formation, implementation, and review of agreed behavior support plans and risk assessments under the supervision of senior staff and in collaboration with the person, their families, carers, and other professionals.

To lead on the planning and implementation of inclusive and meaningful person-centered programmes of activity, outings, and opportunities for cultural enrichment for people with a range of different disabilities, which promote their mental, physical, social, and emotional wellbeing. Lead on arranging, if required, an escort for medical appointments, hospital admissions, social outings working within the relevant risk assessments.

  1. Organise and assist the people we support to control their day-to-day finances, taking into account financial regulations, the mental capacity act, and undertaking regular financial audits and best interest decision making in regard to peoples' monies.

  2. Complete formal supervisions of Residential Care Workers, Activity Organisers, and Domestics.

Manage and lead the staff group with responsibility for allocating, organizing, and prioritizing tasks relevant to the day-to-day care and support of people using the service and efficient running of the home. This includes sickness absence management, rota management, induction. Undertake competency checks of staff and report any concerns to the Registered Manager/Deputy Manager.

To contribute to the recruitment, selection, probation, and induction of new staff, and induction and management of volunteers within the service. Assisting/demonstrating moving and handling techniques, observations, and competency checks to ensure compliance and identify needs for further training.
