Job Description Summary:
We have an exciting opportunity to join our team as a 1-1 residential care worker on a part-time permanent basis for 32 hours a week.

The main duties of this role are to provide 1-1 care and support to meet individuals physical, emotional and social needs based on their individual support plan to keep them safe and well.

You must have direct experience of caring for adults either in a paid or voluntary setting.

It is essential you have good communication skills and have the ability to listen empathically and act on individual needs and preferences to be able to provide best support.

You will ensure that person’s values, rights choices and dignity are met at all times.

You will need to hold a care certificate, level 2 care apprenticeships or equivalent or the ability and willingness to work towards achieving the qualifications.

This is a post that requires you to work shifts on a rota basis including bank holiday and weekends. We are open 24/7, 365 days a year. ROLE AND PURPOSE

The Adult Social Care vision in Hull is of ‘a life not a service’. Adult Social Care is person centred and focusses on individuals strengths to support them to take control of improving their own health, resilience and wellbeing.

PRINCIPAL ACCOUNTABILITIES:

  1. To promote and safeguard the welfare of children, young people and/or vulnerable adults.
  2. Statutory Obligations - Undertake duties and responsibilities in accordance with the national minimum standards of care issued by relevant bodies e.g. Care Quality Commission (CQC). To provide direct support, guidance, supervision and care which meets the persons assessed needs in accordance with their agreed support plan.
  3. Meet the physical needs of people using our service including washing, bathing, continence care, oral hygiene,dressing, postural management, catheter and stoma care and other personal care tasks which include advice and encouragement to support people to undertake daily living tasks for themselves-: promoting choice, independence, and provide, privacy and dignity at all times.
  4. To support people with behaviours of concern in line with the persons care and support plans, following all relevant legislation, physical intervention training, good practice guidance and least restrictive principles. To contribute to the formation, implementation and review of agreed behaviour support plans and risk assessments under the supervision of senior staff and in collaboration with the person, their families, carers and other professionals.
  5. Working closely with the people we support, their families and other professionals to contribute to reviewing each person’s individual care/support plan and risk assessments, to implement plans to ensure their needs are met and agreed outcomes are achieved. Acting as a key worker/link worker on a day to day basis and attending reviews of the people using our service.
  6. To devise and implement interesting, inclusive and meaningful person centred programmes of activity, outings and opportunities for cultural enrichment for people with a wide range of different disabilities which promote their mental, physical, social and emotional wellbeing and engagement with opportunities both within the home and within their communities. Meet the social and emotional support needs of people using our service by encouraging social interaction and promoting contact with family, carers, friends, advocates, and relevant health professionals including acting as an escort for medical appointments and hospital visits
  7. To participate in the identification of needs and reporting of changes and concerns to a line manager such as changes to moving and handling requirements, changes in health and wellbeing or safety concerns. Oversee and monitor the completion of care and support plans, including the completion of monthly summaries. Write and maintain records e.g. daily diaries, care plan records, handover log sheet, incident forms and financial transactions, to ensure that the records for people using our service are accurate, informative and meet relevant regulations. Undertake daily recording including on digital platforms and monitoring of health and wellbeing such as completion of mood and behaviour charts, epilepsy monitoring charts, food intake, bowel charts etc.
  8. Promote a healthy and safe environment by attending to domestic tasks including washing and ironing of linen, cleaning kitchen, bathrooms and other areas of the home to meet infection control guidance. Report any building/equipment defects directly to the Care Leader on duty.
  9. Cook and serve meals, light snacks and drinks taking into account the dietary and nutritional needs of service users including assistance to eat and drink where required using various methods. Completion of appropriate records relating to food handling e.g. temperature recording, cleaning records.
  10. Assist the Care Leader with ordering, stock control and medication reviews. Administer medication as prescribed by medical practitioner and as per risk assessment/health action plan and universal medication policy. Maintenance of appropriate records such as individual medication records and records for checking medication in and out of the building.
  11. To be aware of and participate in own personal development by attending and contributing to supervision sessions, training courses and other departmental forums. Keep abreast of initiatives and legislation within the care profession by attending in-house and/or external training, undertaking eLearning, shadowing etc. Work collaboratively with colleagues, managers and other health and social care professionals in line with Hull City Councils Competency Framework to support the city plan in achieving objectives to improve the lives of people we support. Demonstrate a flexible and adaptable approach, including utilising various communication tools and techniques, using IT equipment and undertaking dynamic risk assessment for unforeseen events. Work cohesively with new colleagues throughout their induction, enabling shadowing to share skills, knowledge and experience to ensure the best outcomes for the people we support and a supportive working environment.
  12. The Health and Safety at Work etc. Act 1974 and associated legislation places responsibilities for health and safety on Hull City Council, as your employer and you as an employee of the council. In addition to the Councils overall duties, the post holder has personal responsibility for their own health, safety and wellbeing and that of other employees; additional and more specific responsibilities are identified in the Council’s Corporate H&S policy.
