An exciting opportunity has arisen for an enthusiastic and motivated Respiratory Specialist Physiotherapist to join our large, innovative community-based team working across Leicester, Leicestershire and Rutland.

This role will support the ongoing development of the Community Respiratory Service, delivering home-based respiratory assessment, intervention, treatment and support to children and young people with complex disabilities. The service aims to reduce the frequency of acute hospital admissions related to respiratory difficulties and facilitate earlier, planned and safe discharge from hospital.

The successful candidate will demonstrate flexibility, initiative and excellent organisational skills, working effectively as part of a multidisciplinary and multi-agency team. You will be responsible for managing a designated respiratory caseload of patients with complex needs, including those requiring long-term ventilation support. Effective time management and the ability to work autonomously are essential.

You will work within the Diana Community Nursing Team and alongside our colleagues within the acute services at University Hospitals of Leicester NHS Trust (UHL) to deliver high-quality, patient-centred care.

A full UK driving licence and access to a vehicle for work purposes are essential requirements of this post.

Main Duties And Responsibilities

• To work alongside other professionals in co-ordinating, delivering and evaluating the Paediatric Respiratory integrated Nursing and Physiotherapy Service provided in the community setting.
• To work in close liaison with the respiratory MDT Team within UHL and other acute settings, to promote enhanced shared care and transition between acute and community provision
• To work alongside multi professionals in developing this business relationship and being accountable for the on-going development of this service, including providing detailed reports and monthly updates.
• To collect, analyse and present highly complex information regarding data collected and use to forecast future service capacity and demand.
• To use advanced clinical reasoning, critical thinking, reflection and analysis to support assessments and management. To work closely with other members of the MDT within health, education, social services to ensure treatment meets the holistic needs of the child.
• To communicate effectively highly complex information and liaise with patients, families/carers, physiotherapy colleagues, other healthcare professionals and outside agencies regarding patient management and progress.
• To supervise, teach, manage and develop physiotherapy colleagues, students, physiotherapy assistants, nursing colleague’s parents/carers and teachers within the area of expertise, using Evidence Based Practice where possible.
• To be an integral part of the wider multi-disciplinary team and to maintain effective communication links with MDT colleagues, other physiotherapy colleagues, outside agencies, family members and carers, in order to facilitate the provision of a holistic care package to the children on your caseload, including those presenting with highly complex needs, so that the best possible clinical outcomes can be achieved.
