Registered Nurse (RN) Role

The Registered Nurse (RN) will participate in the inter-disciplinary care of a caseload of patients whose needs are being met within the intermediate care pathways. This will include shift coordination, team leadership, case management, and direct care delivery in a variety of settings.

The RN will act as a named nurse in the assessment, planning, implementation, and evaluation of nursing care for an identified group of named patients in keeping with the RDaSH definition of Named Nurse. This will be achieved by following evidence-based, individualized plans of care.

Responsibilities

  • To be responsible for the delivery of person-centered plans of care, e.g., assisting with activities of daily living, MDT goal setting, admission & discharge planning, administration of medication, etc., on a daily basis.
  • To be responsible for the leadership of the care team on a shift basis to ensure the safe and effective care delivery of a group of patients through delegation and shift / team coordination.
  • To be responsible for the assessment, planning, implementation, and evaluation of individualized evidence-based care with minimal ongoing supervision of the ward sister.
  • To work as part of the MDT in ensuring the safety of patients to include specific risk assessment and risk management planning.
