Job Opportunity: Pathway Navigator for Rheumatology Patients

An exciting opportunity has arisen for a crucial admin role as a Pathway Navigator for patients under the care of Rheumatology. This role will be the central point of contact for the patients, helping to ensure the individual needs of the patient are supported and that care is built around those needs.

Key Responsibilities

  • Provide direct support for patients to maintain their engagement in the surveillance pathways ensuring that they receive their surveillance scans on schedule.
  • Work closely with the Rheumatology CNS and wider Rheumatology team.
  • Maintain effective communication with and on behalf of the patient, signposting to other resources and information as defined by patient need.
  • Liaise with staff and services to ensure tests and appointments are scheduled.
  • Maintain and populate tracking lists for both nurse-led and consultant clinics.
  • Track patients through their RTT pathway (you must have excellent IT and data entry skills).

Location

As this is a central contact point, you will be predominantly based on site at Royal Lancaster Infirmary.

About the Organisation

We operate from three main hospitals:

  • Furness General Hospital (FGH) in Barrow
  • Royal Lancaster Infirmary (RLI)
  • Westmorland General Hospital (WGH) in Kendal

Additionally, we have a number of community healthcare premises including:

  • Millom Hospital and GP Practice
  • Queen Victoria Hospital in Morecambe
  • Ulverston Community Health Centre
