Job Opportunity: Community Nursing at Newhaven Base

We have a great community nursing opportunity at our Newhaven base in East Sussex. This post would suit a Registered Mental Health Nurse (but we will consider Learning Difficulties nurses who have sufficient experience of working with Dementia or Severe Mental illness.)

You would be a key part of our multi-disciplinary service for older adults with mental health problems, including Dementia. This is challenging work but we receive very positive feedback from our service users and carers and Sussex Partnership NHS Foundation Trust has recently achieved Outstanding for the care it provides.

Role Overview

This post will involve working with older People with complex mental health needs as well as physical health care needs. You will be expected to support both the client and their carers. You will also need to be able to work cohesively with other services both within our Trust, and with other partner agencies.

You will need to be caring, compassionate and a good communicator, with a commitment to improving the lives of our service users through creative, evidence based practice. As a care co-ordinator, you will be responsible for working collaboratively with service users and their carers on your caseload to create care plans that meet their needs and maximise their independence.

You will be based at Hill Rise, Newhaven but the ability to travel is essential for this role.

The post-holder would join the existing Lewes intensive home support team who work to provide intensive home support for older adults in a state of acute mental ill health to prevent deterioration and need for hospital admission. This post will also help support the roll out of the virtual ward into the Lewes neighbourhood mental health team, and is line with the Trust goal of preventing inpatient admissions for older adults wherever possible.

Working Hours

Please note this role is rota based and cover is required 8am - 7pm Monday - Friday and 9-5pm Saturday & Sunday.
