Position Summary

The weekend hospice registered nurse works Friday 8am-4:30am, Saturday 7am-7:30pm, and Sunday 7am-7:30pm, and is included in the on-call schedule on a rotating basis. The weekend hospice RN is responsible for triaging, providing skilled nursing care, assessments, symptom management, education, and support to hospice patients and their families. The RN works collaboratively with the hospice team to ensure continuity of care and implementation of the hospice plan of care.

Duties and Responsibilities

  • Completes routine, PRN, follow-up, recertification, admission, and post-mortem visits
  • Performs comprehensive and focused nursing assessments and evaluates changes in the patient’s condition
  • Provides effective symptom management, in collaboration with the hospice medical director, consistent with the hospice plan of care
  • Triages and responds to urgent patient and caregiver needs
  • Evaluates reported changes in condition and determines the appropriate nursing intervention, including an in-person visit when clinically indicated
  • Provides education to patients and caregivers regarding medications, symptoms management, disease progression, safety, and end-of-life care
  • Ensures medications, medical supplies, and DME are addressed during weekend coverage
  • Performs hospice evaluations and admissions as assigned, and is available to Bristol Hospital to assess and admit hospital patients to GIP hospice services if appropriate
  • Provides clear and thorough handoff communication regarding weekend events, changes in condition, new orders, and follow-up needs

Qualifications

  • Current and unrestricted RN license in the state of CT
  • Current BLS CPR certification
  • Ability to work independently and exercise sound clinical judgement in the home and community setting
  • Strong assessment, communication, organization, and documentation skills
  • Ability to respond effectively to changes in patient condition, and end-of-life symptom management needs
  • One or more years of nursing experience, preferred
  • Previous home health or hospice experience, preferred
