Programmes of care will be developed in partnership with Patients, Family, Carers and Significant others, and will be evidence based taking into consideration the lifestyle, race, gender and cultural background, and factors which impact on physical, mental or emotional health.

The post holder will work in partnership with multi-agency colleagues, individuals, the local community, health improvement staff and other agencies to identify and address health needs as a member of the health improvement workforce.

The post holder will identify and utilise appropriate resources to support families. The post holder will be familiar with and ensure implementation of local, regional and national policies, procedures and guidelines pertinent to their area in the pursuit of the highest standard of care. Assist junior members of staff in planning, priority-setting and decision-making to ensure that they meet client/family needs. Provide leadership and act as a role model for junior members of the team by representing the values and beliefs of the nursing profession as well as that of the organisation. The post holder will be responsible for the day-to-day management of their own workload as delegated by the Band 6, Clinical Nurse Specialist/Team Leader. To participate in systems of practice support, caseload supervision and personal development plans. The post holder will attend staff meetings and clinical and mandatory training. The post holder is responsible for ensuring that they practice within the Legal & Ethical framework as established by Nursing & Midwifery Council (NMC) and National Legislation to ensure the patients’ interests and wellbeing are met.

Knowledge, training, qualifications and/or experience required to do the job

• First level registration with Nursing and Midwifery Council
• Evidence of continuing professional development
• IT skills-word processing/email/internet
• Good communication skills (both verbal and written)
• Ability to work on own initiative
• Problem-solving skills
• Ability to work as part of a team
• Relevant experience working with children / young people
• Car driver
