Job Description
Adult Physical Health Services Overview
Adult Physical Health services are divided into four categories: Anticipatory, Planned and Unplanned Care, and Specialist services. All services aim to improve outcomes for people and maximize access to services through integrated working.
Working alongside the District Nursing Service with an Anticipatory Care focus, the aim is to reduce pressure on planned and unplanned care services. Supporting, teaching, and providing advice and support following best practice guidance is key to the role. To optimize patient care and ensure that where possible, patient independence is preserved and maintained.
Community Teams
The Community Teams aim to maintain people within their own homes/communities wherever possible and facilitate earlier discharge from hospital through delivering a more coordinated approach to care.
The Community Teams work closely with their colleagues in General Practice and the Third Sector organizations. Central to the philosophy and approach, the Community Teams place the individual at the center and proactively maximize opportunities for people to have choice, personalization, and self-care.

Role Details

Self Management Facilitators within the Community Teams aim to maintain people within their own homes/communities, clinics, and hospital inreach wherever possible and facilitate earlier discharge from hospital through delivering a more coordinated approach to care.
The post holder will work within standard operating procedures/pathways to manage their own workload independently but with reference to a supervisor within the clinical area. In doing so, they will support the assessment of patients and implementation of programmes of care ensuring that they practice within their sphere of competence and knowledge. The post holder will work within a range of standard operating procedures and pathways in which they have received training to be competent.

Key Responsibilities
- Actively works towards developing and maintaining effective clinical and corporate working relationships both within and outside the Trust including other agencies, individuals, and higher education Institutes
- Explores the potential for collaborative working and takes opportunities to initiate and sustain such relationships
- Actively contributes to the multidisciplinary team supporting the patient including communication with external care providers, voluntary support services, and other agencies; acting as a credible source of information for other agencies/professionals involved in the care as required
- Works as part of a multidisciplinary team including nurses, occupational therapists, healthcare assistants, medical staff, and pharmacy staff.
