About The Role And Our Ideal Candidate

The purpose of the job is to support the development of a joined-up offer of support for children with SEND and their parents from a centrally located SEND Family Hub in Sutton and to manage and further develop the Preparation for Adulthood Hub.

You will develop a programme of activity from the new SEND Family Hub that is aligned with universal/targeted provision for parents and children with SEND but is focused upon those families who have children with significant levels of need. This will require an evaluation of government guidance about good practice in respect of parental support and groups for children and a needs assessment of current provision. You will also be evaluating and further developing the preparation for adulthood sessions available for young people and their parents and carers at the PFA Hub in Sutton, and managing this programme. You will supervise a parent/child support worker who will offer front-line support at the SEND Family Hub and support the project development of the SEND Family Hub.

In addition to the essential criteria, we are seeking a candidate who is passionate about ensuring a positive experience and outcome for children who have SEND and their parents. You need to be energetic, form strong working relationships with families and professionals, and be able to bring people together to develop a joined-up commitment and offer for families that is consistent and timely. You need to have excellent management and planning skills and have the ability to see plans through to good outcomes.

About Us

The SEND Family Help and PFA Practice Lead will sit under the line management of the Designated Social Care Officer and within the SEND Family Help Service. The SEND Family Help Service is a service that consists of the DSCO and three family help teams, including one social work team that support children and families with disabilities through either targeted or specialist services and direct work.

Over the next 12 months, we will be expanding and developing a physically co-located SEND Family Hub and Help Service in partnership with Public Health, Education, Health and Children’s Services.

The SEND Family Hub and Help Service and PFA Hub will ensure that families with SEND have access to support at the earliest opportunity and are supported on their journey into their child’s adulthood. The SEND Family Hub and Help Service and PFA Hub will bring together services to support families in the community and ensure integration and strong partnership between services.
