About the Role
We have an exciting opportunity within our Acute Inpatient Older Adult Dementia Service for a Senior Acute Care Nurse to join our Team. We are looking for an experienced and passionate clinician who is self-motivated, compassionate and willing to work as part of a team, with a can-do attitude to be successful in this role.
Ward Overview
Langworth Ward is an 18 bedded acute assessment and treatment ward for older adults experiencing mental health problems associated with Dementia (We do take younger individuals depending on diagnosis). The ward is challenging and fast-paced, offering excellent opportunities for professional development.
Responsibilities
As a Senior Acute Care Nurse, you will be responsible for:
- Coordinating the shift and supporting the delivery of excellent nursing care.
- Working closely with the Multi-disciplinary Team including Consultants, Physical Health Nurses, Therapy Team, Psychology, Matron and Advanced Clinical Practitioner.
- Clinically leading and managing a team of staff nurses and healthcare assistants when on shift.
- Supporting the Ward Manager with some operational duties and help develop and maintain clinical standards.
- Demonstrating a capacity for clinical excellence, a high level of emotional resilience, and supporting the ward with developing and enhancing services.
- Risk assessing and managing complex and challenging behaviours of acutely unwell patients and implementing Reducing Restrictive Practice to manage these presentations.
- Ensuring and undertaking the assessment, planning, evaluation and review of individualised plans of care in conjunction with the patient.
- Completing assessments of needs and delivering individualised care to support a successful transition and discharge to the appropriate environment when their recovery has been optimised.
- Being visible and accessible to patients, families and carers working together to ensure efficient, effective and caring services.
- Possessing sound clinical skills and the ability to formulate collaborative care plans.
- Demonstrating a high standard of written and verbal communication.
- Working alongside service users and carers/relatives to continually work towards improving and maintaining high standards.
