Senior Care Act Assessor
We are offering an exciting opportunity for a full-time Senior Care Act Assessor within a well-established and dynamic initial response team.
The position is within the older persons with physical disability team, which is a community-based team working with Adults over 18 years of age with a range of physical and mental health needs.
You will hold a level 3 diploma in adult care or lead adult care/social care worker and/or level 4 diploma in adult care or lead practitioners in adult care level or be willing to complete/achieve this if a level 3 qualification is already held and will also have experience working within a relevant field.

Main Duties of the Job
- You will be expected to access social care and health records to gather information, therefore it is important that you can proficiently use a computer and have an understanding of the importance of good recording.
- You will require effective communication and written skills and be willing to work as part of a team to promote best outcomes for individuals.
- You will be required to be self-motivated and organized in order to manage any competing demands and be provided with all relevant training and support in the role.

We are dedicated to ongoing Social Care development, and you will have access to the Social Work Learning Academy in order to promote professional development. You will also receive the support of your team and your line manager who will provide regular supervision both on a formal and informal basis.
