About the Role:
We are seeking an experienced Senior Care Coordinator, responsible for the day-to-day operational management of a defined portfolio of complex healthcare packages. Reporting to the Operations Manager, the postholder has delegated responsibility for ensuring safe, effective, person-centred and operationally efficient service delivery.
The role acts as the primary operational link between clients, families, commissioners, clinical teams, recruitment and the wider organisation, while providing day-to-day leadership to the care teams supporting allocated services.
Key Responsibilities:
- Manage complex healthcare packages, ensuring safe, compliant and person-centred care in line with contractual and regulatory requirements.
- Triage operational enquiries, incidents and escalations; monitor risks and implement contingency arrangements to maintain continuity of care.
- Act as the primary contact for clients, families and representatives, building trusted relationships and coordinating communication across teams.
- Coordinate package mobilisation and ongoing changes, ensuring workforce and service readiness.
- Lead rota management and workforce planning, coordinating cover, forecasting staffing needs and escalating workforce risks.
- Work with the Clinical Team to support safe delivery of delegated healthcare tasks in line with care plans, risk assessments and clinical guidance.
- Monitor package performance against KPIs and service standards, taking corrective action and escalating risks to performance or viability.
- Ensure efficient delivery of commissioned hours through effective workforce deployment, minimising agency use, overtime and unnecessary costs.
- Maintain accurate client, workforce and operational records, ensuring incidents, complaints and safeguarding concerns are promptly documented.
- Ensure service delivery complies with SD-007, CQC regulations and organisational policies, supporting audits and implementing agreed actions under SD-009.
- Respond promptly to incidents, service disruption and safeguarding concerns, taking appropriate action and working with partner agencies.
- Fulfil Duty of Candour requirements, supporting investigations, implementing learning and maintaining business continuity.
- Apply the Mental Capacity Act 2005 to support informed decision-making and least restrictive care, escalating concerns around capacity, DoLS/LPS and restrictive practice.
- Build effective relationships with commissioners, case managers, MDTs and partner agencies, contributing to governance, service improvement and shared learning.

Outcomes & Performance Measures:
- Safe, compliant and person-centred delivery maintained across the allocated portfolio.
- Commissioned hours, rota coverage and continuity standards achieved, with workforce risks escalated promptly.
- New packages mobilised with confirmed workforce, operational and service readiness.
- Operational incidents, complaints and safeguarding concerns recorded, managed and escalated within required timescales.
- Package KPIs, service standards and agreed corrective actions delivered.
- Accurate and timely client, workforce and operational records maintained in relevant systems.
- Avoidable agency use, overtime and operational costs actively controlled through effective workforce deployment.
- People-management standards met, including objective setting, attendance management, performance management and accurate HR-system records.

Background and Qualifications:
Required:
- Experience coordinating or managing regulated health or social packages of care, ideally complex healthcare packages delivered in the community.
- Experience of rota management, workforce planning and maintaining continuity of care.
- Experience as a Senior Care Coordinator or in a comparable role, coordinating field-based care teams and ready to take broader responsibility for service delivery.
- Working knowledge of safeguarding, incident management, the Mental Capacity Act 2005 and person-centred care.
- Strong communication, stakeholder-management, organisation and problem-solving skills.
- Ability to maintain accurate records, use care-management and HR systems, manage competing priorities and escalate risk appropriately.
Desired:
- Relevant health, social care or management qualification.
- Knowledge of CQC requirements, Duty of Candour and community complex-care governance.
- Experience of package mobilisation, audit activity and continuous service improvement.
