The Role:
As a Senior Case Worker, you will support adults with care and support needs to improve their health, wellbeing, and independence through strengths-based reviews and personalized smoking cessation support. Working with people who may face barriers to accessing mainstream services, including those with disabilities, long-term conditions, mental health needs, and complex social circumstances, you'll build trusted relationships and empower individuals to make positive, lasting changes that support a healthier future.
Working within the Younger Adults Team, you'll collaborate with Social Workers, Public Health colleagues, and partner organizations to embed smoking cessation into social care practice. Through compassionate, non-judgemental conversations, tailored interventions, and effective partnership working, you'll help individuals understand the impact of smoking on their health and wellbeing, access stop smoking services and wider community support, and contribute to the ambitions of Smokefree Hampshire 2030.
What you’ll do:
- Complete assessments, reviews, and community-based visits, identifying opportunities to support people to improve their health, wellbeing, and independence through smoking cessation.
- Use strengths-based and motivational approaches to help people overcome barriers to quitting smoking and achieve positive outcomes.
- Make referrals to local stop smoking services, provide practical advice, and signpost individuals to relevant support and community resources.
- Use person-centred approaches to raise awareness of the benefits of stopping smoking, encouraging informed choices and engagement with evidence-based support.
- Promote awareness of smoking cessation support across the service, sharing learning with colleagues and contributing to preventative practice and continuous improvement.

What we’re looking for:
- Strong communication skills, with the ability to engage people in sensitive, non-judgemental conversations about health and wellbeing that encourage positive behaviour change.
- Experience of working with adults with care and support needs, building positive relationships, and using strengths-based approaches to motivate people to achieve their personal goals.
- Understanding of the impact of smoking on health, wellbeing, quality of life, and wider associated risks.
- Knowledge of the Care Act 2014, Mental Capacity Act 2005, and how strengths-based practice can support positive outcomes and greater independence.
- Commitment to promoting positive health and wellbeing, championing smokefree lifestyles, and empowering people to make informed decisions about their health.

Travel Requirements:
You will be required to travel to various locations within Hampshire and the Isle of Wight or other local authority, and it is essential that you have a current driver’s licence and access to a reliable vehicle on a regular basis.
