Role Description
The Senior Clinical Pharmacist will provide remote, part-time clinical pharmacy services to primary care practices, working closely with general practitioners and other healthcare professionals. Day-to-day responsibilities include:
- Conducting structured medication reviews
- Optimizing prescribing
- Providing evidence-based advice on medicines
- Supporting safe, cost-effective treatment plans

The role involves:
- Reviewing patient records
- Managing repeat prescriptions
- Contributing to long-term condition management
- Supporting medicines reconciliation and deprescribing where appropriate
The Senior Clinical Pharmacist will:
- Participate in clinical audits
- Help implement prescribing guidelines
- Support quality improvement initiatives
- Contribute to patient education on medication safety and adherence
The position requires:
- Effective communication with practice staff
- Accurate documentation in clinical systems
- Adherence to local and national medicines management policies
Qualifications
- Candidates should possess strong clinical pharmacy skills in medication review, prescribing optimization, and medicines reconciliation.
- Candidates should possess skills in evidence-based practice, clinical guideline interpretation, and therapeutic decision-making.
- Candidates should possess effective communication, counseling, and collaboration skills for working with patients and multidisciplinary teams.
- Candidates should possess skills in clinical documentation, use of electronic health records, and remote consultation platforms.
- Candidates should possess organizational and time-management skills to handle caseloads, prioritization, and follow-up tasks in a part-time remote setting.

Relevant qualifications include:
- Registration as a licensed pharmacist with the appropriate regulatory body
- Substantial post-qualification clinical experience in primary care or similar settings
A postgraduate qualification or credential in clinical pharmacy, primary care pharmacy, or independent prescribing is highly beneficial.
Familiarity with primary care prescribing systems, medicines optimization initiatives, and quality improvement processes is an advantage.
