Pharmacist for South Kent Coast Community Mental Health Team (CMHT)
We are looking for an enthusiastic, motivated, and committed pharmacist to provide high-quality clinical pharmacy service to the South Kent Coast Community Mental Health Together team (CMHT)

This exciting role was created as part of the Kent and Medway Community Mental Health Transformation Programme designed to support patients, with severe mental illness (SMI), live fulfilling independent lives in the community.
Key Requirements:
- Independent prescribing qualification or be working towards it
- Post-graduate qualification in pharmacy
- Mental health experience

- Take on operational responsibility for the safe and effective delivery of medicines management services within their Community Mental Health Together Team.
- Facilitate pharmacy-led clinics and reviews to support, monitor, and review patients requiring advice and guidance in regards to prescribed medications.
- Network and drive innovation in mental health medicines optimisation across the primary and secondary care interface, supporting people with mental health conditions closer to their communities and homes.
- Work in partnership with GPs, primary care pharmacists, and pharmacy technicians, in addition to KMMH pharmacy team and clinicians in secondary care.
