Our Integrated Network Teams
Our Integrated Network Teams are made up of nurses, therapists, and support workers who work in an integrated way to support service users on their healthcare journey. We would love for you to come along for the ride.
Key Responsibilities
- Complete holistic assessments of service users.
- Develop and deliver person-centred care plans.
- Line manage junior staff and contribute to their development through supervision, 1:1s, and training.
- Undertake and complete the appropriate training, supported by a robust competency framework, within the time allocated, in order to carry out the requirements of the role and deliver a competent clinical service.
- Communicate with the senior leadership team in the locality to ensure smooth delivery of service.
- Identify risks and act accordingly to minimise risk to self, service users, and colleagues in line with Organisational policy and procedures.
- Coordinate shifts and delegate/allocate care to the team in accordance with policy.
- Act as a point of escalation for staff to report to on discovering any issues or deteriorations.
- Undertake further training as required in order to develop extended skills and enhance patient care.
