Haemoglobinopathy Liaison Nurse Role
It is an exciting time to join the Haemoglobinopathy team at Sandwell and West Birmingham NHS Trust as we look forward to a new working model of integrating community and hospital Sickle Cell and Thalassaemia Service.
The team is looking for a friendly and dynamic nurse to join the team in a liaison nurse role, an integral part in facilitating the transition between paediatric services, but increasingly bridging the gap between hospital and home too.
Responsibilities
- Experience of caring for people with haemoglobinopathy conditions or awareness of the challenges they face.
- Excellent communication skills.
- Competent in venepuncture, cannulation, and the use of CVADs.
- Prescribing qualification and clinical assessment qualification are essential.
- All aspects of care of the haemoglobinopathy patient: screening and counselling on diagnosis.
- Clinically supporting patients with their well being in an outpatient setting but also supporting during inpatient stays, delivering day treatments and managing the unwell patient.
- Coordinating and participating in regional MDTs, teaching and raising awareness throughout hospital and community.
- Visiting patients in their own homes to deliver a holistic approach to care.
- Supporting the Lead nurse with service development.
