Band 6 Community Mental Health Nurse

The Band 6 Community Mental Health Nurse is a key member of the West Norfolk Community Mental Health Team (CMHT), providing specialist assessment, care planning, treatment, and care coordination for adults experiencing severe and enduring mental health difficulties. Working within a multidisciplinary team, the post holder will deliver high-quality, person-centred care that promotes recovery, supports independence, and enables individuals to achieve the best possible outcomes within their local community.

This position is open to band 6's or band 5's who have completed their preceptorship.

• Deliver high-quality, recovery-focused mental health care in line with NSFT values, Trust policies, and the NMC Code.
• Undertake assessments under appropriate supervision, contribute to care planning, deliver interventions, and evaluate outcomes for service users.
• Monitor mental and physical health needs, including medication management, escalating concerns where appropriate.
• Develop and manage a caseload under supervision, working collaboratively with service users, carers, and partner agencies.
• Undertake risk assessments and contribute to risk management and safeguarding processes.
• Maintain accurate clinical records and contribute to effective multidisciplinary team working.
• Participate in supervision, appraisal, mandatory training, and continuous professional development activities.
• Contribute to quality improvement, audit, and service development initiatives.
