Band 7 – Senior Mental Health Practitioner
An exciting opportunity has arisen for a Band 7 Senior Mental Health Practitioner to work for the South Manchester Home-Based Treatment Team based at Laureate House, Wythenshawe Hospital.
About the Role
The post holder will work collaboratively with the Team Manager, Senior Mental Health Practitioners, and colleagues across Urgent Care Services, Mental Health Liaison, Home Treatment Teams, inpatient wards, and the Patient Flow Service, to deliver the Trust’s aims of providing effective care, support, and recovery to individuals with serious mental health problems who are experiencing a crisis in their mental health. The post holder will support individuals to progress safely and effectively through urgent care pathways, promoting recovery, reducing unnecessary delays, and ensuring person-centred care throughout their journey.
Key Responsibilities
- Work collaboratively with colleagues across various services to deliver the Trust’s aims.
- Contribute to the maintenance and monitoring of high standards of urgent care.
- Ensure the Trust’s vision, values, and principles are consistently upheld.
- Participate in out-of-hours Operational on-call duties (subject to approval).
- Develop effective partnerships with primary care colleagues and non-statutory service providers.
- Provide leadership and management for all staff in the HBTT.
- Contribute to the management of team processes for accepting and allocating referrals.
- Ensure effective and planned interventions are delivered.
- Support individuals in moving through the urgent care pathways effectively and being supported in their recovery.
- Undertake assessment, care planning, therapeutic intervention, and review of care for individuals experiencing a crisis.
- Provide an advisory resource to colleagues.

Working Conditions
- 37.5 hours a week including shifts, weekends, and bank holidays.
- Service operates over 24 hours, 7 days a week.
