The MACCT

The MACCT is lead jointly by Whittington Health NHS and Haringey GP federation. It is a leading example of this integrated working and one of North London’s earliest neighbourhood‑based care models. Bringing together professionals from health, social care and the voluntary sector to support adults with frailty or complex needs, MACCT is widely recognised as the model on which the Integrated Neighbourhood Team (INT) can be built, demonstrating what truly joined‑up, preventative and person‑centred neighbourhood care can achieve.

Colleagues appointed to this role will join an ambitious and inclusive partnership shaping the future of integrated care in Haringey. You will work closely with clinicians, community partners and system leaders to support residents in a more connected, preventative and person‑centred way. These posts are funded for one year (until 31/3/27) through neighbourhood test‑bed funding, providing an opportunity to build on the MACC model and support the development of a model Integrated Neighbourhood Team (INT).

Responsibilities

  • To collaborate with partner agencies in the development of the local delivery of services provided by the MACCT.
  • To deputise for MACCT Services Manager as required.
  • In conjunction with senior managers, participate in recruitment, selection and appointment of staff.
  • To provide formal practice and performance management supervision to MACCT/NLFT members in accordance with multi-disciplinary supervision policies.
  • To provide specialist professional supervision to members of own profession.
  • To undertake yearly staff appraisals and develop Individual Development Plans for MACCT/ NLFT Staff Members.
  • To build and nurture links between MACCT and Primary Care and other agencies with the NLFT Mental Health Services (Including but not exclusive to; OP CMHT and Memory service).
