Are you a dynamic, experienced Occupational Therapist with a passion for providing excellent care to residents in care homes and enhanced assessment beds?
An exciting opportunity has arisen for an experienced band 5 or established band 6 Occupational Therapist to strengthen the Care Homes Team at Sandwell and West Birmingham NHS Trust. We cover Sandwell care homes, Harvest View, and other Enhanced Assessment Beds across the borough.
About the Role
As part of your role, you will ensure the best quality therapy for residents in both care homes and enhanced assessment beds, delivering effective treatment and rehabilitation. You will support the Team Lead Occupational Therapists in establishing and developing pathways and services provided to those in enhanced assessment beds and care homes. This will support the delivery of specialist assessment, rehabilitation, and discharge planning. You will work as part of an established multidisciplinary team including Physiotherapists, Occupational Therapists, Therapy Support Workers, ACPs, Nurses, and Speech and Language Therapists to meet the complex needs of our patients. The care homes team provides a 7-day therapy service to our patients.
Key Responsibilities
- Ensure best quality therapy for residents in both care homes and enhanced assessment beds, delivering effective treatment and rehabilitation.
- Support the Team Lead Occupational Therapists in establishing and developing pathways and services provided to those in enhanced assessment beds and care homes.
- Formulate and deliver patient-specific Occupational therapy programmes based on patients' goals and needs, including groupwork sessions such as breakfast groups and exercise classes.
- Evaluate patient progress, reassess, and alter treatment programmes if required, and set patient-centered goals.
- Support the discharge process by working as part of a multidisciplinary team to facilitate discharges from Harvest View and EAB beds, including completing post-discharge assessments in the individual's own home.
- Attend MDT meetings and provide feedback on patient plans and progress.
- Provide effective daily guidance and supervision for junior staff, including Support Workers.
- Be a clinical educator and support students undertaking placements within the team.
