Role Overview
In your new role, you will work closely with Physiotherapists, Assistant Practitioners, support staff, district nurses, specialist nurses, and social workers to provide the best possible care to service users in their own homes.
Service Details
We are working towards a 7-day service, 8 am to 8 pm on a rota basis. Self-rostering is encouraged.
An exciting opportunity has arisen to join our developing integrated community services, providing clinical triage, rehabilitation, falls management, palliative care support, and support timely discharges from our acute Trust partners.
Key Responsibilities
- Work autonomously as well as part of the multi-disciplinary team, liaising closely with health and social care colleagues to coordinate services, promote recovery from illness, and enable health, wellbeing, and self-care.
- Visit patients within their own homes using a goal-orientated focus to maximize patient's independence, safety, and quality of life.
- The post will involve cross-site working and community visits.

Requirements
- A full UK driving license and vehicle for business use is required for this post; however, reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.
