Senior Physiotherapist - Urgent Community Response Team

Band 6: £39,959-£48,117

37.5 Hours Per Week

Permanent

Responsibilities

An excellent opportunity has arisen for a Senior Physiotherapist to join our Urgent Community Response team.

We are seeking an experienced and motivated Physiotherapist with a strong community background to join our dynamic and forward-thinking team.

The ideal candidate will demonstrate a positive attitude, excellent communication skills, and the ability to work flexibly and collaboratively to meet the needs of the Urgent Community Response Team.

The Successful Candidate Will Be Expected To

  • Provide comprehensive assessment, treatment, and evaluation of patients referred to the service during periods of crisis.
  • Deliver short-term intervention programmes aimed at stabilising patients and preventing deterioration.
  • Contribute to the avoidance of unnecessary hospital admissions through timely and effective care.
  • Manage a caseload of adults aged 18+ with complex health and social care needs in the community.
  • Support individuals to achieve their maximum level of independence by improving physical and mental health outcomes, enhancing coping capacity, recommending appropriate equipment and interventions and working closely with multidisciplinary teams to ensure holistic, patient-centred care.
