About the Role
We are seeking an enthusiastic, experienced, and highly motivated Physiotherapist to join our Urgent Community Response Service. You will be a HCPC-registered Physiotherapist with demonstrable experience in admission avoidance, rehabilitation, and supporting patients to maximise their independence, self-manage their conditions, and remain safely within their own homes.
Key Requirements:
- Excellent communication and interpersonal skills
- Ability to engage effectively with a diverse range of patients, carers, and healthcare professionals
- Well organised, able to work independently, manage a varied caseload, and prioritise competing demands in a fast-paced environment
- Experience across a range of clinical settings is desirable
- Sound understanding of the opportunities and challenges associated with delivering care in community-based services
Responsibilities:
- Passionate about delivering high-quality, person-centred care
- Committed to working collaboratively within a multidisciplinary team to prevent unnecessary hospital admissions and support timely recovery and rehabilitation
- Undertake comprehensive, holistic assessments and provide evidence-based interventions tailored to the needs of this patient group
Leadership Role:
- Play a key leadership role in the planning, coordination, delivery, and evaluation of rehabilitation programmes
- Responsible for supporting and supervising junior staff and students
- Foster collaborative working across a range of clinical teams and settings

Essential Requirements:
- A car driver with access to a vehicle for work as you will be expected to travel across the North East Essex area

- Deliver person-centred, goal-focused rehabilitation that empowers patients to become active participants in managing their own health and wellbeing
- Enable patients to remain safely and independently within their own homes
- Demonstrate strong relationship-building skills, embrace innovation and integrated working
- Committed to delivering high-quality rehabilitation alongside Consultants, Advanced Clinical Practitioners, Nurse Practitioners, Occupational Therapists, Physiotherapists, Associate Practitioners, and Community Support Workers to achieve the best possible outcomes for patients
