Community Nurse Opportunity in SLWR
The post is open to Community Nurses with the District Nursing Specialist practitioners qualification; or this post could be an ideal career development opportunity for a motivated and passionate individual who enjoys working in rural communities.
The postholder will provide care in Community/District Nursing within a defined geographical area. Holding responsibility for the clinical caseload and supports the Integrated Team Manager in his/her leadership role. You will ensure the leadership and management of the nursing team along with the quality and standard of care delivered.

The qualified Community Nurse is a specialist community practitioner who provides holistic care and promotes independent living. You will work closely with the multidisciplinary team supporting care home, carers, and voluntary sector and care at home staff, link practitioners and colleagues to enhance their knowledge and skills in health care management promoting avoidance of hospital admission where appropriate and supporting / facilitating early discharge.
Requirements
- Full UK driving licence and access to a vehicle is essential for this role along with over 2 years of community nursing experience.
