Starting Salary: £32,578 progressing to £35,570 per annum
Hours: 37 hours per week (10:00am to 18:00pm)

About The Team
The Transfer of Care Hub is a multi-disciplinary team made up of health and social care staff who support system flow in the East & North Herts area. The Senior Progress Chaser plays a vital role in keeping hospital discharges moving efficiently across the system. Working at the heart of the Transfer of Care Hub (TOCH), the role will track patient progress through hospital, rehab, and community pathways—helping to make sure no discharge gets delayed and every patient transitions safely and on time.
About The Role
The Senior Progress Chaser plays a pivotal role in driving system flow and supporting the smooth transfer of care across hospital and community settings. This role ensures real-time monitoring of patient progress, proactive problem-solving to remove discharge barriers, and close liaison with acute hospital leads to maintain oversight of every referral and discharge.
This fast-paced role involves chasing outstanding actions, and linking in with ward staff, community teams, and care providers to ensure everything needed for a discharge is in place. You’ll be the go-to contact for tracking and updates, making sure that every piece of the process is coordinated and recorded in real time.
As part of the development of the Transfer of Care Hub (TOCH) this position supports a 7-day operational service, providing consistent oversight and escalation across multiple sites and system partners.
The Senior Progress Chaser ensures that every step in the discharge process is tracked, delays are swiftly addressed, and accurate progress intelligence is shared daily and monthly to support flow and capacity management.
Key Responsibilities:
- Drive and maintain system-wide flow by tracking and monitoring patient journeys across hospitals, community settings, and discharge pathways (0–3).
- Proactively identify, investigate, and escalate issues or delays to keep discharge timelines on track.
- Maintain real-time visibility of discharges across different sites, ensuring transport, care packages, and placements are in place for safe and timely transitions.
- Support the delivery of a 7-day service, ensuring continuity and consistency across all partner sites, including acute hospitals, rehab units, and community services.
- Build and sustain effective relationships with acute leads, local authority teams, and external care providers to streamline pathways and resolve operational blockages.
- Provide analytical insight through data tracking, report production, and performance monitoring to inform senior managers and drive continuous improvement.
- Contribute to reducing delayed discharges and improving hospital flow through proactive engagement, coordination, and problem-solving.
- Manage the assistant progress chaser and will liaise with families/representatives and other discharge staff across all departments.

About You
Essential:
- Previous experience and Knowledge of hospital discharge and community care pathways.
- Experience working in health or social care settings.
- Strong organisational and communication skills.
- Confident using IT systems and tracking tools to maintain real-time visibility.
- A proactive problem-solver who thrives in a fast-moving environment.
- Ability to work weekends and to be based at Lister hospital and the role will require frequent physical visits to the ward.
