Eligibility Notice

Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.

About the Role

The Community Nursing Service delivers personalised care to housebound patients aged 18+ with complex and long-term conditions. Our aim is to support patients to remain safe, well, and independent in their chosen environment, preventing avoidable hospital admissions through proactive, needs-led care.

Responsibilities

  • Working within teams aligned to Primary Care Networks, you will be part of a multidisciplinary team delivering holistic assessment, care planning, and evaluation in partnership with patients and carers.
  • Care includes palliative and end-of-life support, medication administration (including IV therapy), wound care, bladder and bowel care, and health promotion.
  • You will manage a clinical caseload, undertake comprehensive assessments, and deliver high-quality, evidence-based interventions to meet complex patient needs while promoting independence and long-term condition management.

Key Duties

  • As a senior clinician, you will coordinate care delivery, triage referrals, and allocate work based on staff competencies, ensuring safe and effective practice.
  • You will lead decision-making for complex cases, develop integrated care programmes, and represent the team within MDT and Primary Care Network meetings.
  • You will play a key role in supporting and developing staff through supervision, appraisals, and clinical guidance, acting as a role model and mentor.
  • You will also contribute to quality improvement, ensuring CQC standards are met, and support performance management to drive service excellence.
  • In addition, you will oversee daily handovers, review care plans, and deputise for senior leaders when required.
  • Ongoing professional development is essential, including completion (or working towards) the Specialist Practitioner District Nurse qualification.
  • The service operates 7 days a week, 08:00–18:00, with some flexibility available.
