Job Description

The post holder will undertake the planning and delivery of direct patient care within a community mental health or hospital setting and to discharge statutory community care functions on behalf of the organisation.

To be an active member of the integrated multi-disciplinary team, ensuring the provision of social care services, participating in assessment, triaging, care planning, treatment, evaluation, and crisis intervention/management.

To deliver the provision of Mental Health Care services to people with mental health care needs and provide support, advice and guidance to other health and social care professionals.

Provide an appropriate learning environment for relevant students.

Duties And Responsibilities

  • Promote and provide expertise and guidance to the wider multi-disciplinary team on a range of social care interventions, which acknowledge the health and social context, human rights and self-determination of the individual
  • To observe the requirements of the Mental Health Act 1983 (as amended by Mental Health Act 2007), Mental Capacity Act 2005, Care Programme Approach and related Codes of Practice, and all other legislation affecting your role and duties as a qualified social worker.
  • Demonstrate good knowledge of Safeguarding Policy and practice in relation to Adult and Child Protection and to attend external case conferences on behalf of the Service (e.g. Child/Adult Protection Case Conferences)
  • Have a thorough working and applied knowledge of the NHS and Community Act 1990, the Mental Health Act 1983 (as amended by the Mental Health Act 2007), Mental Capacity Act 2005, as well as being acquainted with other legislation which may be relevant to the service group (e.g. Children Act 1989 and 2004), Carers Act 2005, National Assistance Act 1948, Human Rights Act 1998, FACS criteria, NCC and V4 Page 4 of 14 NSFT policies and procedures
  • To assess health and social needs as part of an integrated team, and implement/commission packages of care where appropriate
  • To be fully aware of all relevant Policies, Standards, Standing Orders and Procedures, and to observe them in the course of your work
