Senior Social Worker – Community Team

Liquid Personnel is recruiting an experienced Senior Social Worker – Community Team for its client's Community Team based in Shropshire. This is a fantastic opportunity for a skilled Adult Social Care professional to support adults across the North Shropshire locality, delivering strengths-based interventions that promote independence, wellbeing, and positive outcomes.

As a Senior Social Worker, you will work with older adults, people with physical disabilities, and individuals with complex care and support needs. You will manage a complex caseload, undertake statutory assessments, contribute to safeguarding processes, and work collaboratively with partner agencies to help individuals remain safely within their communities.

What You Will Be Doing

  • Undertaking Care Act 2014 assessments, reviews, and support planning for adults and carers.
  • Managing a complex caseload and delivering high-quality social work interventions.
  • Applying strengths-based practice to maximise independence and wellbeing.
  • Developing and reviewing person-centred care and support plans.
  • Contributing to safeguarding enquiries and adult protection procedures.
  • Undertaking Mental Capacity Assessments and supporting Best Interest decision-making.
  • Supporting safe and timely hospital discharges into the community.
  • Coordinating services to prevent unnecessary admissions and promote independent living.
  • Working closely with health professionals, housing providers, carers, and partner agencies.
  • Identifying and managing risk while promoting individual choice and autonomy.
  • Ensuring service users and carers are actively involved in assessment and planning processes.
  • Maintaining accurate case records, reports, and statutory documentation.

What We Are Looking For

  • Social Work qualification (BA, BSc, MA Social Work, DipSW, CQSW, or equivalent).
  • Current registration with Social Work England (SWE).
  • Significant post-qualified experience within Adult Social Care.
  • Strong understanding of the Care Act 2014, Mental Capacity Act 2005, and Adult Safeguarding legislation.
  • Experience undertaking complex assessments, reviews, and support planning.
  • Experience contributing to safeguarding investigations and risk management processes.
  • Hospital discharge and community social work experience.
  • Proven ability to work effectively within multi-agency environments.
  • Excellent communication, report-writing, analytical, and decision-making skills.
  • Full UK driving licence and access to a vehicle is desirable.
