Job Introduction
Job Title: Senior Social Worker
Starting Salary: £44,253 progressing to £48,738 per annum (pro rata for part time) + £2k welcome payment, relocation & car lease schemes (terms apply)
Hospital teams receive an additional market enhancement of between £2,170 and £2,326 plus 5% of annual salary (terms apply)
Hours: 37 hours per week

As a Senior Social Worker you will be responsible for overseeing and supporting the delivery of social services to communities or specific areas.
The role of a Senior Social Worker is crucial in the discharge to assess model, which aims to improve outcomes for people by reducing the length of hospital stays and promoting independence at home. Responsibilities Include
- Coordinating different social workers and support services
- Managing schedules and handling issues or concerns within the community
- Providing emotional support and guidance to individuals and families
- Assessing and reviewing the situations of individuals and families
- Organising and managing packages of support to enable people to lead fuller lives
- Making referrals to other agencies and participating in multidisciplinary teams
- Maintaining accurate records and preparing reports for legal action

Based in Stevenage, the Adult Disability Service supports adults with learning disabilities, physical disabilities and autism to live fulfilling, independent lives. We are passionate about empowering people to achieve their goals, build community connections and make informed choices about their future.
We are seeking a Senior Social Worker with a strong interest in autism and a commitment to developing high-quality services for autistic adults. As the Autism Lead, you will support colleagues to build their knowledge and confidence, promote best practice, and work collaboratively with partners to improve outcomes for the people we support.

East Herts Locality Team
Farnham House, Stevenage

Based Stevenage and working across services within those areas, the Adult Disability Service provides support to adults living with a learning or physical disability.
We aim to enable people to make positive decisions in their lives, promote their independence and ensure they can get involved in their local community. We're a service motivated by and dedicated to social care intervention and community social work.

Integrated Discharge Service
Post Hospital Review Team
37 hours per week | Permanent
Our Integrated Discharge service in the West of Hertfordshire supports an integrated approach to managing the care of people who require support through the admission and discharge pathway, so the role will involve working alongside Occupational Therapists, Community Care Officers, health colleagues and care providers. You will have the ability to undertake innovative assessments and care planning with individuals, carers and their personal support networks. We promote personalisation focusing on the wellbeing of the individual. Through care planning we strive for increased independence, choice and control by enabling people to direct their own support. You will gain extensive experience in the field of adult social care, safeguarding practice and multi-disciplinary working.

Older Peoples Service - West
St Albans Locality Team
Apsley, Hemel Hempstead
37 hours per week | Permanent
The Older Person Service is a dynamic service that works across the community under the Care Act to safeguard, assess, plan support and review adults and cares with long term health and social care needs. We can offer Hybrid working arrangements, flexible working practices, community engagement and volunteer activities and, as part of our commitment to you, as well as promote your continuous Learning & Development.

Essential
About you
You will be a SWE registered Social Worker, with at least 2 years post qualified experience with a full driving licence.
You will have successfully completed an ASYE program working to the Social Work England professional standards. You will undertake The Care Act assessments for eligible current and historical cases, using our Connected Lives strategy to review and manage packages of care.
You will also actively undertake a specialist role, e.g. AMHP, Best Interest Assessor or Practice Educator, have a place secured on a course, or commit to undertaking this as soon as appointed.
This is a fast-paced, community-focused environment. Hybrid working is available, with a minimum of 2 days per week in the office.
