Senior Social Worker - Hospital Discharge Team

Our client based in Coventry is seeking an experienced Senior Social Worker to join their Hospital Discharge Team. This is an excellent opportunity for a highly skilled practitioner to support adults through timely, safe, and effective hospital discharge pathways, ensuring individuals receive the right support to maximize independence and achieve positive outcomes within their communities.

The successful candidate will work as part of a multidisciplinary hospital-based service, managing complex cases involving adults with a range of health, social care, and support needs. You will provide professional leadership, support less experienced practitioners, and work closely with health colleagues, community teams, carers, families, and partner agencies to prevent delayed discharges and promote strengths-based outcomes.

This role requires a confident practitioner who can balance complex decision-making, risk management, safeguarding responsibilities, and statutory duties whilst ensuring person-centred practice remains at the heart of all interventions.

Key Responsibilities

Core duties include:

  • Managing a complex caseload of adults requiring social care intervention within hospital settings.
  • Undertaking Care Act 2014 assessments, reviews, and support planning to facilitate safe and effective discharge from hospital.
  • Supporting adults and their carers through strengths-based, person-centred interventions that promote independence and wellbeing.
  • Coordinating discharge arrangements, ensuring appropriate care, support services, equipment, and community resources are in place.
  • Undertaking Mental Capacity Assessments and contributing to Best Interest decision-making processes where required.
  • Managing safeguarding concerns and undertaking safeguarding enquiries in accordance with relevant legislation and local procedures.
  • Working collaboratively with hospital clinicians, discharge coordinators, therapists, community teams, care providers, families, and carers.
  • Assessing and managing risk effectively, ensuring safe discharge planning and positive outcomes.
  • Identifying opportunities for reablement, rehabilitation, and preventative support to reduce long-term dependency on services.
  • Providing professional guidance, mentoring, and support to less experienced social workers, students, and colleagues within the team.
  • Chairing and participating in multidisciplinary meetings, discharge planning meetings, and safeguarding discussions where required.
  • Maintaining accurate, timely, and legally compliant assessments, reports, case records, and support plans.
  • Supporting service improvement initiatives and contributing to the ongoing development of best practice within hospital social work services.

Qualifications & Experience

  • Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
  • Registration: Must be registered with Social Work England.
  • Experience: Significant post-qualified experience within Adult Social Care, including hospital discharge and complex case management.
  • Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, Continuing Healthcare processes, and associated statutory guidance.
  • Hospital Experience: Proven experience working within hospital discharge pathways, acute settings, or multidisciplinary health environments.
  • Assessment Skills: Excellent assessment, care planning, risk management, and decision-making skills.
  • Safeguarding Experience: Experience managing safeguarding concerns and undertaking safeguarding enquiries involving vulnerable adults.
  • Leadership Skills: Experience supporting, mentoring, or guiding less experienced practitioners and contributing to professional development.
  • Multi-Agency Working: Strong experience working collaboratively with health professionals, community services, care providers, and partner agencies.
  • Communication Skills: Excellent communication, negotiation, report-writing, and organisational skills.
  • Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
