Adult's Senior Social Worker
Liquid Personnel is recruiting an Adult's Senior Social Worker to join its client’s Hospital Social Work team, based in Warwickshire. This is an excellent opportunity for a highly skilled practitioner to work within a county-wide hospital service, supporting adults with complex care and support needs through safe, timely, and person-centred discharge planning.
The successful candidate will work closely with health partners, multidisciplinary teams, carers, families, and community services to assess needs, coordinate support, and ensure individuals achieve positive outcomes following hospital admission. Working within a fast-paced environment, you will provide professional leadership, support complex decision-making, and contribute to the delivery of high-quality Adult Social Care services through an established Discharge to Assess model.
This role requires a confident Senior Social Worker with strong assessment skills, extensive knowledge of Adult Social Care legislation, safeguarding, hospital discharge pathways, and Continuing Healthcare processes.
What will your responsibilities be?
- Managing a complex caseload of adults requiring social care support within acute and community hospital settings.
- Undertaking Care Act 2014 assessments and determining eligible care and support needs in partnership with individuals, carers, and health professionals.
- Supporting safe and timely hospital discharge through effective discharge planning and implementation of Discharge to Assess pathways.
- Developing, coordinating, and reviewing person-centred Care and Support Plans that promote independence, wellbeing, and choice.
- Defining, designing, and coordinating support packages using statutory, voluntary, and private sector services within available resources.
- Undertaking Mental Capacity Assessments and facilitating Best Interest decision-making in accordance with the Mental Capacity Act 2005.
- Supporting and overseeing safeguarding enquiries, ensuring vulnerable adults are protected from abuse, neglect, and exploitation.
- Working collaboratively with multidisciplinary health and social care teams to manage transfers of care and reduce delayed discharges.
- Supporting Continuing Healthcare (CHC) processes and working alongside health colleagues to ensure appropriate funding and support arrangements are considered.
- Providing professional guidance, mentoring, and practice support to less experienced practitioners within the team.
- Participating in duty systems, referrals, multidisciplinary meetings, safeguarding discussions, and case reviews.
- Maintaining accurate, timely, and legally compliant case records, assessments, reports, and support plans.
- Contributing to service development, best practice, quality assurance activities, and continuous improvement initiatives.

Qualifications and Experience:
To be successful in this role you must have;
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Adult Social Care, including complex case management and hospital-based social work.
- Hospital Experience: Experience working within Hospital Social Work Teams, Discharge to Assess Services, Integrated Discharge Teams, or similar Adult Social Care settings.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, Continuing Healthcare processes, and associated statutory guidance.
- Assessment Skills: Proven ability to undertake complex assessments, reviews, support planning, and risk management interventions.
- Safeguarding Experience: Experience undertaking safeguarding enquiries and managing complex safeguarding concerns.
- Leadership Skills: Experience providing professional support, mentoring, coaching, or guidance to colleagues.
- Multi-Agency Working: Strong experience working collaboratively with NHS professionals, community services, care providers, carers, and partner agencies.
- Communication Skills: Excellent communication, report-writing, analytical, negotiation, and decision-making skills.
- Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
