Liquid Personnel is recruiting Senior Social Worker for its client's Short Term and Reablement Team (STAR) at Wokingham.
This is an excellent opportunity for a highly skilled practitioner to support adults with complex care and support needs, promoting independence, wellbeing, and positive outcomes through strengths-based and person-centred practice.
The successful candidate will work with individuals and their carers to undertake complex assessments, reviews, safeguarding enquiries, and risk management interventions in accordance with the Care Act 2014. The role focuses on maximising independence, preventing or reducing the need for long-term support, and supporting adults to remain safe and well within their communities. You will also provide guidance, supervision, and professional support to colleagues whilst contributing to service development and quality assurance across the team.
What will your responsibilities be?
- Undertaking complex Care Act 2014 assessments, reviews, and reassessments, ensuring eligible needs are identified and addressed appropriately.
- Supporting adults and carers through strengths-based and person-centred approaches that promote independence, wellbeing, dignity, and individual choice.
- Developing, implementing, and reviewing personalised Care and Support Plans that reflect assessed needs, outcomes, and aspirations.
- Completing and overseeing risk assessments and risk management plans, balancing safety with positive risk-taking and independence.
- Acting as Safeguarding Adults Manager (SAM) where required, overseeing safeguarding enquiries and supporting less experienced practitioners undertaking investigations.
- Identifying and responding to safeguarding concerns, ensuring appropriate action is taken in accordance with multi-agency safeguarding procedures.
- Completing Mental Capacity Assessments and contributing to Best Interest decision-making processes where required.
- Supporting the use of assistive technology, Telecare, and other innovative solutions to maximise independence and reduce reliance on long-term support.
- Managing a complex caseload whilst supporting the wider team in prioritising work and responding to competing service demands.
- Working collaboratively with health professionals, voluntary organisations, care providers, police, commissioning teams, finance colleagues, and other stakeholders to achieve positive outcomes.
- Chairing professional meetings, safeguarding meetings, case conferences, MDTs, and other case management discussions as required.
- Preparing and presenting detailed reports, assessments, recommendations, and funding requests for management consideration.
- Maintaining accurate, timely, and legally compliant records using electronic case management systems.
- Providing supervision, mentoring, guidance, and professional support to registered and non-registered Adult Social Care professionals.
- Contributing to service development, continuous improvement initiatives, and the promotion of best practice across Adult Social Care.

Qualifications & Experience:
To be successful in this role you must have,
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, or CQSW.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Adult Social Care, including complex assessment, safeguarding, and risk management.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Adult Safeguarding legislation, and associated statutory guidance.
- Leadership Experience: Experience supervising, mentoring, supporting, and developing colleagues within Adult Social Care.
- Safeguarding Experience: Proven experience managing safeguarding enquiries and acting within safeguarding leadership roles.
- Assessment Skills: Excellent assessment, analytical, decision-making, and report-writing skills.
- Multi-Agency Working: Proven experience working effectively with health services, care providers, voluntary organisations, police, and other partner agencies.
- Technology Awareness: Understanding of assistive technology, Telecare, and other preventative approaches that promote independence.
- Communication Skills: Excellent interpersonal, presentation, negotiation, and stakeholder management skills.
- Professional Practice: Commitment to strengths-based, person-centred practice and continuous professional development.
