Service Coordinator - Doncaster Supported Living
📍 Base Location: Doncaster Office (covering services across the region)
💷 Pay Rate: £13.52 per hour
🕒 Hours: Full-time, 40 hours per week (Monday to Friday)

As a Service Coordinator, you'll be the organisational backbone of our services across the region. Working closely with the Registered Manager, you'll keep rotas, care plans, audits and records accurate and up to date, so our teams can focus on delivering safe, high-quality, person-centred support.
What You'll Be Doing
Administration and Coordination
- Providing administrative support across a region of bespoke Supported Living care packages, making sure every service runs smoothly and consistently.
- Supporting the Registered Manager with rota management: planning shift cover, monitoring staffing levels, managing annual leave and sickness, and making sure every package has the right staff with the right skills.
- Keeping service records, staff files and documentation accurate, organised and audit-ready.
- Coordinating operational tasks such as food orders, weight tracking, healthcare matrix updates and appointment scheduling.

Care Planning and Oversight
- Reviewing, updating and maintaining bespoke care plans and risk assessments so they reflect each person's current needs.
- Supporting the creation and review of Positive Behaviour Support (PBS) plans.
- Maintaining strong communication with families, healthcare professionals and multidisciplinary teams (MDTs).
- Promoting person-centred planning that encourages independence, inclusion and wellbeing.
Audits and Compliance
- Completing audits across services, including medication, care plan, health and safety and fire safety audits.
- Carrying out Nourish compliance checks and weekly audits, and following up on any actions.
- Conducting spot checks across supported living homes to monitor quality and standards.
- Supporting the Registered Manager to prepare for CQC inspections and maintain regulatory compliance.
- Supporting medication management, including ordering, auditing and staff competency checks.
Team Support
- Supporting and mentoring Shift Leaders and Support Workers, modelling best practice.
- Assisting with staff supervisions, team meetings and performance reviews.
- Taking part in staff recruitment, interviews and competency assessments.
- Providing on-call support on a rotational basis.

What We're Looking For
- A compassionate, proactive individual committed to improving lives.
- Excellent organisational and administration skills, with the ability to manage competing priorities across multiple services.
- Experience with rota management, care planning and completing audits in a care setting.
- Experience supporting adults with learning disabilities, autism and/or mental health needs.
- A minimum of 3 years' experience in a support work role, including experience in a senior or supervisory position.
- Confident IT skills; experience with digital care systems such as Nourish is desirable.
- Strong communication skills and the confidence to work alongside managers, families and external professionals.
- A full UK driving licence and access to a vehicle, as the role covers services across the region.
