Urgent Community Response (UCR) Services
Urgent Community Response (UCR) Services are an integral part of the Urgent and Emergency Care pathway within Northamptonshire and therefore have a primary focus on avoiding hospital admissions and facilitating early discharge from hospital.
About the Role
This role is an exciting opportunity to work with dedicated clinical teams and our external health and social care partners to deliver this pathway and contribute to the development of the UCR Services within it.
The services provide urgent, time-limited, multidisciplinary community-based interventions. Delivery methods include home visiting and remote monitoring.
Timely and effective Urgent Community Response services are high on the national agenda and a clear understanding of these services and their contribution to the national and local objectives is essential.
We want to create effective and high-performing teams and so we are looking for somebody who is proactive and has the proven leadership skills to provide that direction and oversight.
Responsibilities
- To be responsible for operational performance including contractual, financial, quality and safety and people, ensuring that all contractual and Trust obligations are met for Urgent Community Response services.
- You will support the Directorate Leadership Team to devise service plans for the future delivery of services and advise on the operational impact of service developments.
- To take overall management responsibility for service delivery and operational matters for Urgent Community Response Services
- To take a lead role in the development and reorganisation of teams when required;
- Review and develop service models, including developing clinical structures, workforce planning, including recruitment and training, and operationalising the service;
- To assist with the continual modernisation, redesign and implementation of creative solutions that support National and Local initiatives, leading on as required;
- To develop robust interagency relationships with key agencies supporting the development of integrated care pathways;
- To ensure Service Users and Carers are included in all the work the Recovering Independence Services do.
- The post holder will be expected to work flexibly and will be part of the "on-call" and "out-of-hours" arrangements for the Trust.
- To provide operational leadership holding responsibility for operational delivery of service outcomes.
- To leadership and manage the talent pool across services.
