Role Description

We are looking for an experienced Service Transition Manager for a 3-month hybrid contract role to lead the successful transition of IT services and projects into business-as-usual operations. The role requires close collaboration with project teams, technical teams, vendors, and business stakeholders to ensure operational readiness, effective risk management, and seamless service handover with minimal business disruption.

Mode of hiring: Fixed term employment contract (3 months)

Working Model: Hybrid (3 days from office)

Location: Lincolnshire, UK

Key Responsibilities

  • Plan and manage end-to-end service transition activities.
  • Develop and maintain transition plans, schedules, and governance processes.
  • Ensure service readiness through documentation, knowledge transfer, and support handovers.
  • Coordinate with project, infrastructure, application, and support teams throughout the transition lifecycle.
  • Identify, track, and mitigate transition risks, issues, and dependencies.
  • Facilitate go-live readiness reviews and post-implementation support activities.
  • Ensure compliance with ITIL, change management, and organizational standards.
  • Provide regular status updates and transition reports to stakeholders.

Required Skills & Experience

  • Proven experience as a Service Transition Manager, Transition Lead, or similar role.
  • Strong knowledge of ITIL Service Transition and change management processes.
  • Experience managing IT infrastructure, application, cloud, or managed service transitions.
  • Excellent stakeholder management, communication, and coordination skills.
  • Strong risk management and problem-solving abilities.
  • Experience with service acceptance, operational readiness, and knowledge transfer activities.
  • Ability to work effectively in a fast-paced hybrid environment and manage multiple priorities.
