Exciting Opportunity for Clinical Triage Nurses

An exciting opportunity has arisen for an experienced, enthusiastic and motivated nurse to join our hard working team!

We are looking for Clinical Triage Nurses to join the Dudley Clinical Hub (Single Point of Access) offering telephone Triage, clinical advice and signposting.

About the Role

Based at Brierley Hill Health and Social Care Centre, the successful applicant will join an integrated, collaboratively focused team, employing a proactive clinical practice and knowledge to provide phone triage / clinical assessment skills in order to support urgent care for patients across our health economy.

The Service is the single point of access for patients registered with a Dudley GP for all admission avoidance referrals from professionals and for patients requiring Health and Social Care Services.

Working Hours:

  • 8am – 9pm, 7 days per week, and 365 days per year
  • Flexibility required

The service offers extensive support and knowledge to enable patients to be managed away from the acute setting and to implement the right level of care support to achieve this.

Key Responsibilities

  • Provide clinical support, assistance and professional expertise to all Healthcare professionals.
  • Support in avoiding inappropriate hospital attendances and admissions.
  • Act as the referral point for the Urgent Community Response Team.
  • Take telephone referrals from GP’s, West Midlands Ambulance Service, Community Nurses and all other Health Professionals.
  • Visit patients in their own home if required.
