3 Month Contract With A Local Authority

Job Purpose

To work as part of a multi-skilled, multidisciplinary team to support the assessment, care management, and support needs of vulnerable adults and their carers. The postholder will promote independence, choice, control, health, and wellbeing through person-centred approaches, enabling citizens to access appropriate support, services, equipment, and community resources to meet their assessed needs.

Key Responsibilities

  • Contribute to the assessment of the needs of vulnerable adults and their carers, supporting specialist and more complex assessments where appropriate.
  • Contribute to effective support planning, enabling citizens and carers to exercise choice, make informed decisions, and maximise their independence.
  • Support citizens to promote their health, wellbeing, choice, and control, with due regard to mental capacity.
  • Undertake allocated care management duties and contribute to the effective review and monitoring of support arrangements.
  • Communicate and engage effectively with citizens, families, carers, and other stakeholders using a person-centred approach.
  • Work closely with colleagues across multidisciplinary and multi-skilled teams to ensure coordinated and effective support.
  • Work collaboratively with community agencies and professionals to help citizens access appropriate resources and services.
  • Promote independence and support citizens to remain connected to their communities and lead fulfilling lives.
  • Identify, with appropriate training, the need for simple assistive equipment during assessments and arrange provision where appropriate.
  • Identify, with appropriate training, the need for more complex equipment or adaptations and make referrals to the relevant specialist services.
  • Maintain accurate and timely records relating to assessments, care management, support planning, and service provision.
  • Participate in individual and team development activities, training, supervision, and continuous professional development.
  • Work flexibly across the service and contribute to service improvement and effective partnership working.

Requirements

  • Experience of working with vulnerable adults within a social care, health, community, or related support environment.
  • Understanding of person-centred approaches and the principles of choice, control, independence, and self-directed support.
  • Experience contributing to assessments, support planning, care management, or the delivery of care and support services.
  • Good understanding of the needs and rights of vulnerable adults and their carers.
  • Understanding of mental capacity principles and their application within care and support settings.
  • Ability to communicate effectively and sensitively with citizens, families, carers, professionals, and partner agencies.
  • Ability to work effectively as part of a multidisciplinary and multi-skilled team.
  • Strong organisational skills, with the ability to manage allocated responsibilities and maintain accurate records.
  • Ability to identify support needs and make appropriate referrals to specialist services.
  • Willingness to undertake relevant training relating to assistive equipment, adaptations, assessment, and care management.
  • Commitment to promoting independence, wellbeing, inclusion, and positive outcomes for vulnerable adults.
  • A full driving licence is required for the role.

  • Hours per Week: 37.00
  • Location: Walsall, West Midlands
  • Working Arrangements: Hybrid and office working, with a minimum of 2 days office based
  • Driving Licence: Required
  • Payment: Bi-Weekly
