We are seeking an enthusiastic and motivated Social Care Practitioner to join our busy and supportive Long-Term Adult Social Work Team based in the beautiful High Peak area of Derbyshire.
This is an exciting opportunity for someone who is passionate about supporting adults to achieve positive outcomes, promote independence, and live safe and fulfilling lives within their communities.
As part of a dedicated multidisciplinary team, you will work with adults with a range of care and support needs, undertaking strengths-based assessments, reviews, development of support plans to ensure people receive the right support at the right time.
About The Role
As a Social Care Practitioner, you will:
- Manage a varied caseload within a long-term adult social care team.
- Undertake strengths-based assessments and reviews.
- Work closely with individuals, families, carers, and partner agencies.
- Support initial safeguarding enquiries and risk management processes.
- Develop and review support plans that promote independence and wellbeing.
- Contribute to positive outcomes through person-centred practice.
- Work collaboratively within a supportive and experienced team environment.

About You
We are looking for someone who:
- Has experience of working within health, social care, or a related field.
- Is committed to delivering high-quality, person-centred support.
- Has excellent communication and partnership-working skills.
- Can manage competing demands and work effectively in a busy environment.
- Demonstrates a strong understanding of safeguarding and risk management.
- Is able to work independently whilst contributing positively to team objectives.
- Has good IT and recording skills.
