Salary: £33,835 - £39,278 per annum
Contract: Permanent, Full-time, 36 hours per week
Working hours: Monday to Friday
Location: Bromley Civic Centre
Working pattern: Hybrid (minimum 2 days per week office attendance)

We are seeking three experienced Social Care Practitioners to undertake planned reviews for adults receiving support from the London Borough of Bromley.
Working as part of the Coordination and Review Team, you will be responsible for carrying out assessments and reviews in accordance with the Care Act 2014. This will involve reviewing the needs and support arrangements of adults residing in care homes, Shared Lives schemes, Extra Care Housing, or those receiving domiciliary care services through Direct Payments or commissioned care packages, including individuals placed in residential care settings.
The role may require travel both within and outside the borough to visit and review individuals in their current placements.

Key Responsibilities
- Complete timely and proportionate Care Act needs assessments and Annual reviews for residents at home, Placements and Extra Care Housing scheme.
- Support residents, ensuring care and support plans reflect the agreed outcomes following the Care Act Assessments.
- Work collaboratively with care providers, housing and health professionals to ensure continuity of care.
- Identify risks associated with changes to care provision and put appropriate risk management and safeguarding measures in place.
- Undertake Mental Capacity Assessments and contribute to Best Interest decision-making where required.
- Ensure that individuals, families, and advocates are fully informed and supported.
- Maintain accurate, timely, and defensible case recording in line with statutory and organisational requirements.
Essential Requirements
- Experience of working in Care Management settings
- Recent experience in adult social care, ideally with older people and/or housing-based support services.
- Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding duties.
- Experience of completing assessments and reviews for people in receipt of a Bromley funded care package in their own homes, Extra Care, supported housing, residential and nursing Placements, or similar settings.
- Confidence working autonomously and managing a complex caseload.

Desirable
- Experience supporting service transitions and hospital discharges or provider changes.
- Experience of partnership working with housing providers.
