Job Details
Undertaking a range of personal/physical care tasks which may include assistance with washing, dressing, eating, toileting, administration of medication and any other tasks in accordance with the care plan.
A requirement of this post is that you must be able to complete training and maintain safe practice in Crisis Prevention Institute physical intervention programme. This training will be provided.
Providing positive behaviour support and encouragement for people to make their own choices and decisions in all aspects of their daily lives. We are looking for someone who will focus on what someone can do rather than what they can’t.

The individual should possess a high level of emotional resilience to enable you to meet service user needs which may be challenging at times.
Linking with other providers, statutory and voluntary agencies, including health, to develop positive working relationships compiling information on other relevant and available community resources.
Auditing, documenting and reporting all required checks e.g. finance, health and safety, cleaning and maintenance of equipment/aids.
Operating within the framework of service policies and procedures e.g. Health and Safety Regulations, Data Protection Legislation and Confidentiality Policy, assessing potential risks to yourself, colleagues and service users.
Assessing, planning, implementing, monitoring and reviewing of individual support plans
Actively promoting and ensuring a high standard of care practice in accordance with National Care Standards, ensuring individuals are always treated in a respectful and dignified manner.
Keeping accurate records of outcomes focused support planning and review for the people whom you support
Undertaking the keyworker role - monitoring and ensuring the service user is being offered the opportunities, experience, care and support identified in their care plan.
Organising, chairing and attending staff and other relevant meetings, contributing to service development and finalising reports.
