We are delighted to offer three permanent full-time Social Worker opportunities within the Amber Valley South Adult Social Work Team. We are seeking motivated, person-centred practitioners who are passionate about making a positive difference to the lives of adults and their families.
Amber Valley South is a fast-paced and rewarding locality where no two days are the same. Our team works with adults who have a wide range of care and support needs, delivering strengths-based assessments, reviews, safeguarding enquiries, support planning and multi-agency partnership working to achieve the best possible outcomes.

About The Role
You will work with adults who require support to maintain their independence, improve their wellbeing and achieve their personal outcomes.
Key responsibilities include:
- Undertaking strengths-based assessments and reviews.
- Developing person-centred care and support plans.
- Working collaboratively with families, carers, health colleagues, providers and partner agencies.
- Supporting safeguarding activity and risk management.
- Contributing to Continuing Healthcare assessments and attending Decision Support Tool meetings.
- Promoting independence through community resources, assistive technology and informal support networks.
- Participating in team duty arrangements.

About You
You will:
- Hold a recognised Social Work qualification and be registered with Social Work England.
- Have a sound understanding of the Care Act 2014, Mental Capacity Act 2005, safeguarding legislation and associated guidance.
- Be committed to delivering high-quality, person-centred practice.
- Possess excellent communication, organisational and IT skills.
- Be able to manage competing priorities within a busy environment.
- Demonstrate resilience, professional curiosity and a commitment to reflective practice.
- Be able to work independently whilst contributing positively to team working.
