Join our Mental Health Discharge Team!

This dynamic role focuses on supporting individuals in acute psychiatric wards. You will play a vital part in assessment, discharge planning, and enabling timely, person-centred transitions from hospital to home. Your expertise will help individuals regain their independence and improve their mental health outcomes.

As a Social Worker, Your Key Responsibilities Will Include

  • Assessment and Planning: Conduct thorough assessments and create tailored discharge plans.
  • Reviewing S117 cases
  • Collaboration: Work closely with individuals, families, carers, and partner organisations to understand their needs and preferences.
  • Commissioning Care: Identify and arrange necessary care and support for safe discharges.
  • Promoting Independence: Encourage individuals to live independently through reablement and community-based support.

To Thrive In This Role, You Will Need

  • A recognised Social Work qualification.
  • Registration (or eligibility to register) with Social Work England.
  • Familiarity with community care services and relevant legislation in adult social care including the Mental Health Act.
