Job Title: Permanent Social Worker
About the Role
We are seeking to recruit an experienced Permanent Social Worker to join the Hospital Discharge Team based at the Royal Derby Hospital. Experience of working in a hospital setting is beneficial but not essential.
Responsibilities
- Facilitate safe, sustainable transfers of care for people leaving hospital.
- Provide Link Worker support within a community hospital setting.
- Work flexibly across the University Hospitals of Derby and Burton.
- Work alongside the Care transfer hub, which may require you to work at Clay Cross hospital.
- Work a 5 in 7 working pattern but at the present time staff are expected to work on a Monday to Friday basis. If this is to change prior notice will be given in advance.

Requirements
- Ability to manage change.
- Maintain positive relationships with health partners and other agencies.
- Commitment to working in a person-centred way with clients, their families and carers to maintain and maximise their independence, whilst observing the tight timescales under the Care Act and current guidance.
- Flexibility, resilience, adaptability, and willingness to continuously learn and cascade new skills and knowledge.
- Ability to work as part of a multi-disciplinary team and share a clear sense of purpose and vision with resilience and determination to bring about change.
