About the Role

We are looking for an exceptional Social Worker to come to work within our Adult Social Care – Out of County Hospital Team – Lewes & North Wealden, which is based in Lewes. On occasion you may also be asked to undertake work for the Community Hospital Social Work Team which covers the Eastbourne and Hastings areas.

We have excellent relationships with local partners and hospital trusts. Together we work collaboratively to serve people to safely discharge from hospital back to their homes in the community. We promote the home first model and discharge to assess approach and believe with the appropriate safe level of care clients can maintain their independence and stay at home, safely where possible. We work closely with our reablement teams and our inpatient rehabilitation providers to maximise client’s independence and build confidence and positive outcomes for the client we work with. Where necessary we work with our local Integrated Commissioning Boards (ICB’s) to ensure people we serve get access to continuing care funding.

Requirements

We are looking for candidates who:

  • Can meet the travel requirements of the role. You will be required to travel across East Sussex, including rural areas.
  • Are a Registered Social Worker with Social Work England and will have obtained a social work qualification
  • Have completed your Assessed and Supported Year in Employment or the equivalent if prior to 2012, (NQSW year and academic module)
  • Can demonstrate resilience and flexibility to manage a dynamic and complex caseload.
  • Has knowledge of appropriate legal and policy frameworks that informs and mandates social work practice.

In return for your excellent skills and expertise, East Sussex County Council offers extensive training and career development; you will be supported by monthly individual supervision where you can develop your continuous professional development (CPD) via monitoring your work against the Professional Capability Framework and Knowledge and Skills Statement aligned with your level of experience. You will have the opportunity to engage in monthly reflective supervision with your peers to add towards your registration requirements.
