Are you ready to make a real impact? Join our Mental Health Discharge Team as a Social Worker in Adult Social Care. This fast-paced role focuses on supporting individuals in acute psychiatric wards, ensuring timely and person-centred discharge planning. You will work closely with health and social care partners to help people transition smoothly from hospital to home.

As a Social Worker, Your Key Responsibilities Will Include

  • Conducting strengths-based assessments and discharge planning.
  • Collaborating with individuals, families, carers, and partner organisations to understand what matters most.
  • Commissioning care and support to enable safe and timely discharges.
  • Supporting individuals to live independently, focusing on reablement and community-based support.
  • Contributing to improved outcomes for people with mental health needs.

To Succeed In This Role, You Will Need

  • A recognised Social Work qualification.
  • Registration (or eligibility to register) with Social Work England.
  • Knowledge of the Mental Health Act and experience working with mental health.
  • Understanding of community care services and relevant legislation in adult social care.
