Our client based in Cornwall is seeking an experienced Social Worker to join their Adult Care and Support Team.
This is an excellent opportunity for a dedicated practitioner to support adults with a wide range of health and social care needs through strengths-based, person-centred social work practice.
The successful candidate will undertake high-quality assessments, care planning, interventions, and reviews, working closely with individuals, families, carers, health professionals, and partner agencies. You will play a key role in promoting independence, prevention, reablement, and positive outcomes whilst ensuring individuals remain at the centre of all decision-making.
This role requires a confident Social Worker who can manage a varied caseload, undertake complex assessments, manage risk, and deliver high-quality statutory social work interventions within Adult Social Care.
Qualifications & Experience
- Professional Qualification: MA, BA, BSc in Social Work, Diploma in Social Work, CQSW, CSS, or equivalent recognised Social Work qualification.
- Registration: Must be registered with Social Work England.
- Experience: Significant post-qualified experience within Adult Social Care services.
- Legislation Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Human Rights legislation, Adult Safeguarding legislation, and associated statutory guidance.
- Assessment Skills: Proven ability to undertake complex assessments, analyse risk, identify strengths, and develop outcome-focused support plans.
- Safeguarding Experience: Experience undertaking safeguarding enquiries and supporting vulnerable adults.
- Multi-Agency Working: Strong experience working collaboratively with health professionals, partner agencies, carers, and community organisations.
- Communication Skills: Excellent communication, interviewing, negotiation, report-writing, analytical, and decision-making skills.
- IT Skills: Strong working knowledge of electronic case management systems and information technology.
- Driving Licence: Full UK driving licence and access to a vehicle is required due to the community-based nature of the role.
