Liquid Personnel is recruiting for a Social Worker to join its client’s Adults Discharge to Assess Team in Bristol
This is a compelling opportunity to support citizens during a pivotal stage of their care, ensuring safe and effective transitions from hospital to community-based care.
About the Role
Help shape improved hospital discharge processes by assessing, supporting, and advocating for adults during critical periods of transition from hospital to community settings.
Responsibilities
- Assess and support citizens across three discharge pathways post-hospital discharge
- Conduct post-discharge assessments and reviews adhering to Care Act frameworks
- Perform Care Act Assessments (CAA) while providing targeted support and discharge planning
- Execute Best Interest (BI) decisions in alignment with the Mental Capacity Act (MCA)
- Manage safeguarding concerns in line with statutory obligations
- Provide duty cover when required
- Participate in Quality Assurance (QA) panels to support funding approvals

Qualifications & Experience
Essential:
- Qualified Social Worker with a valid Social Work England registration
- Strong background in Adults Services, preferably in discharge coordination or hospital-based teams
- Deep familiarity with Care Act, Mental Capacity Act (MCA), and safeguarding legislation
- Exceptional ability to operate efficiently in fast-paced environments
