Job Title: Social Worker
Starting Salary: £39,481 progressing to £43,149 pa (pro rata for part-time) + £2k welcome payment, relocation & car lease schemes (terms apply)
Hours: 37 hours per week

Older Peoples Service - West
Prevention of Admissions Team
Location: Hemel Hempstead
Hours: 37 hours per week | Permanent
The South West Prevention of Admission Team supports individuals who live in their own home or usual place of residence and are aged 65 or over, or under 65 with a confirmed dementia diagnosis, who are experiencing a deterioration in health or care circumstances that is likely to lead to a hospital, residential, or inpatient admission within 48 hours.
The team covers South and West Hertfordshire, including the localities of St Albans, Dacorum, Watford, Hertsmere, and Three Rivers.

The Older Person Service is a dynamic service that works across the community under the Care Act to safeguard, assess, plan support and review adults and carers with long term health and social care needs. Adult Disability Service East
Stevenage Team
Hours: 37 hours per week | Fixed Term to January 2027
North Herts Team
Hours: 37 hours per week | Permanent
The Adult Disability Service provides support to adults living with a learning or physical disability. We aim to enable people to make positive decisions in their lives, promote their independence and ensure they can get involved in their local community. We're a service motivated by and dedicated to social care intervention and community social work.
About You
Essential:
- Social Work degree
- Registration with Social Work England
- Full Driving licence

Driving Required
You’ll need to have the ability to drive and/or travel around Hertfordshire, either using your own mode of transport or a company car to travel to other locations.
English Fluency
The ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post (for those whose language is a signed language the provision of a sign language interpreter who speaks English to the necessary standard of fluency will be required).
